1. Asked my bday twice, where you already have that data from app. 2. The button were grayed out but still functional in survey makes it unclear. 3. These all questions are not clear as they come one after the other, we as a user assume there would be only one rating. If you have multiple questions you can ask in one page one below the other with just one CTA and done.
